New Faculty Awards Program
The nomination form for the New Faculty Awards Program is a Word template document. The document will need to be downloaded, completed, printed and attached to each copy of the nomination. Below are the instructions to download this document and to complete the nomination form.
- Click on the link below holding down the "control" key on your keyboard.
- A dialogue box will appear. Select: "Save Linked File As ..."
- A dialogue box may appear prompting you to select the name and destination of the file. The file name should be "NF.dot" The file name should not include quotes or any additional and appended phrase (i.e. ".txt").
- Nomination form for the New Faculty Awards Program
- Double click the file (NF.dot) on your desktop. The document will open in Word.
- Data entry fields are marked with gray highlights. Use the "tab" key to advance to the next data entry field. The following information is required to complete the data entry:
- nominee's information (first name, last name, title, appointment date, department, telephone number, street address, city, state, zip, and e-mail address)
- nominee's academic background (Baccalaureate and Ph.D. institution, department, year, and advisor; if applicable, first postdoctoral institution, years, and advisor; and second postdoctoral institution, years, and advisor)
- contact information for the nominator and the three individuals selected to write letters of support
- abstract and title of research topic
- Print the nomination form.
- The Nomination should arrive as a hard copy at the Foundation office on or before May 8, 2008. The mailing address for the Foundation is:
